An Encrypted End-to-End Process
Hoarding is often a solution to a problem people don’t know they have.
The aim of journey mapping an individual who has hoarding issues is to better understand the triggers, what support they did or didn’t have and how we can begin to address wider support for this mental health condition.
The WHO (World Health Organisation) recognised Hoarding as a mental health disorder which will now bring about change in approaches to supporting a person(s) who hoards.
The Clouds End Referral Form works with the Empowering- Communities’ Hoarding Referral and Case Management Solution to enable Housing providers, Local Authorities, Fire & Rescue Services, the Police and their partners a means to take their management of referrals, engagement and support of vulnerable people who hoard to a new level. The process helps all practitioners and the public to identify those at risk and improve outcomes by enabling support to be wrapped around them quickly and effectively.

Filtering and Automatic Referrals
Create detailed multi-partner reports without having to compile data from multiple systems.
The desk gives you the ability to filter warnings by a range of different criteria. For example, a practitioner may wish to identify the warnings relating to drink/drugs or mental health issues. In addition, the desk can automate a number of processes, such as referral to partner organisations. This means that you can trigger a home visit automatically when a set criteria is met. Or maybe you would like to send all youth-related warnings to an appropriate service.
Produce reports and highlight trends
Many practitioners and analysts often have to battle with manual forms and several spreadsheets when trying to run reports. As the management desk dovetails neatly into E-CINS’ case management system, reports are much more readily available. This not only saves time but it also improves accuracy by reducing human error.
